Admission Procedure Luther Memorial Home
- Application: Needs to be turned in to Social Services PRIOR to admission. LMH needs this information in order to prepare for the new resident. The application can be dropped at LMH, mailed, sent via email to strosen@lmhnd.org or faxed to us at 701-786-9022.
- Nursing Visit: A Nurse will come out to assess a potential resident for admission, in their own home or at the hospital, prior to admission to ensure we are able to meet this person’s needs.
- History & Physical: A History and Physical (H&P) must be done within five days prior to admission. We can assist you in making this appointment with the physician of your choice and provide the paperwork to the Dr. that needs to be completed.
- Admission Times: LMH admits new residents at 10AM, Monday through Thursday. Admissions are not done on weekends.
There are exceptions to the admission process so please don’t hesitate to call with questions or if you need clarification on the above information.
Social Services is available to give tours of LMH and answer questions. The Social Service office is open Monday through Friday from 8AM to 4PM.

Moving into LHM?? Consider bringing the following items...
All rooms are supplied with a bed, nightstand, dresser, clock and calendar.
Clothing: 5 to 7 Changes of clothing (machine washable and dryable)
- Shirts/sweaters/sweatshirts - Pants - Socks/knee highs - Undergarments
- Shoes - Slippers with non-skid soles -Robe (for bath day) -Pajamas (3/4 pairs)
**Clothing labeled Dry Clean Only, Delicate, is jeweled or is wool, silk, linen, etc. will be laundered at your own risk. LMH will not be responsible for these types of clothing should they get damaged or accidentally be washed. Because of the water temperature used for washing, items tend to shrink over time. It is helpful to buy clothing one size larger than usual.
BRING ALL HOME MEDICATIONS!
Personal Hygiene items: Items such as toothbrush, toothpaste, comb, deodorant and incontinence products are provided; however, you are welcome to bring your own if you prefer a certain brand.
Items recommended to bring for your room, not provided by LMH: (As space permits)
-Recliner -TV and/or radio (Cable Provided) -Fan
-Lamp -Bedspread/Blanket -Pictures/Bulletin/Items to Hang
-Hobby work/reading material -Electric Shaver (males and females)
-Telephone/Cell Phone (Family must contact phone company to set up service as phone service is not provided by LMH.)
NOT ALLOWED:Extension cords, Rugs, Electric blankets, Heating pads, Humidifiers, Microwaves
- Power strips with surge protectors can be used for TV (not cable TV box) and Computer/Printer only per Fire Marshal.
- Storage: Belongings may not be stored on the floor, under the bed, or be kept in a private bathroom unless it is in an enclosed container (dresser or wardrobe) per the State Health Department.
*Some items allowed based on OT evaluation, such as scooters, scissors, Keurig, curling irons, etc. For questions on items, please refer to nursing department or social services.
