JOB SUMMARY:
The primary purpose of the charge nurse position is to provide direct care for residents, carry out physician’s orders, assess resident needs, and supervise CNA staff. The charge nurse assists with planning, organizing, developing, and directing nursing service to comply with current applicable federal, state, and local regulations and standards of practice.
JOB DUTIES RESPONSIBIITES:
The charge nurse is responsible for the overall care and documentation for an assigned group of residents, including medications, treatments, and personal care, in compliance with established care plans and applicable regulations and standards of practice. The following activities are unique to this position:
- Organizes the administration of medications, treatments, diagnostic measures, and resident personal care to maximize safe and complete service to residents.
- Administers and documents medications and treatments. Performs nursing procedures such as catheterization, tube feedings, suction, applying and changing dressings, packs, and colostomy and drainage bags; collecting blood, sputum, fecal, urine samples; cares for the body after death. May supervise a medication aide assigned to administer medications.
- Supervises CNAs to complete care required and document appropriately.
- Assesses residents and makes decisions for treatment and activity. Contacts physician or others as necessary.
- Receives and transcribes written, verbal, or telephone orders, records in the resident medical record, and initiates implementation. Calls pharmacy/faxes for new orders; communicates need for special equipment or supplies to supply manager.
- Notifies physician and resident family members for change of condition, accident, or incident at the earliest opportunity. Document in the medical record, and, as necessary, on an incident report form.
- Receives medical and other information from providers, family members, and vendors and follows through as necessary.
- Receives and gives report between shifts, to ensure continuity of care and service for residents.
- Reports irregularities in orders, medications, supplies, equipment to the appropriate practitioner or department manager.
- May participate in quality assurance, infection control, safety, and product committees.
- Works cooperatively with all staff to ensure each resident attains or maintains the highest practicable level of physical, emotional, and social well-being.
- Maintains adequate supplies and directs economical and efficient use by all staff members. Adheres to policies for notification of supply and medication usage.
- Regularly expects CNAs to fulfil assigned duties and makes that expectation known to the CNAs. Conducts annual evaluations of CNAs supervised.
- Participates in the development, review, and revision of policies and procedures for the department.
- Performs other duties as assigned.
WORKING CONDITIONS:
- Attends all in-facility nursing inservices.
- Maintains active nurse’s license.
- Attends 9 out of 12 in-facility professional nurses meetings each year.
- Required to attend one Coordinated Inservice per year that includes all the inservices topics required by North Dakota regulation.
JOB REQUIREMENTS:
1. Must have a working knowledge of nursing procedures.
2. Must be familiar with policies and procedures of the facility.
3. Must be able to work cooperatively with others.
4. Must be conscientious of importance of accuracy and thoroughness in work.
5. Must have a preference for performing services of benefit and help, for scientific and technical activities, and for understanding and responding to medical problems and concepts.
QUALIFICATIONS:
Education:
- Must hold a Bachelor of Science degree-RN that is a graduate of an accredited school of nursing.
- Must be licensed by the North Dakota Board of Nursing.

The RN is responsible for assessment of health status and analysis of data, development of the care plan, and evaluation of responses to care provided. The RN will base data, assessments, and contributes to the development of the care plan and evaluation of response to nursing interventions.
EXPERIENCE
- Charge nurse must have experience in the supervision of support staff and managing multiple priorities during the course of the day. Must have good English communication skills with residents and staff.
SPECIFIC REQUIREMENTS
1. Must possess verbal skill necessary to communicate with residents, physicians, staff and families of residents.
2. Motor coordination and manual dexterity are necessary to coordinate hands, eyes and fingers in administering medications and treatments, in using clinical instruments, and in handling residents.
3. Clerical perception is required to prepare record and chart and to maintain resident care plans.
4. Must have the ability to perform a variety of duties characterized by frequent change, as work schedule will change on daily basis.
PHYSICAL REQUIREMENTS
1. Work is of medium demand.
2. Stands and walks most of the working day with occasional lifting of residents with assistance.
3. Reaches, handles and fingers instruments and equipment in caring for the residents’ needs.
4. Near-visual acuity is necessary to read gauges and dials on equipment and to administer medications.
5. Good hearing is necessary to distinguish differences in heartbeat and breathing of residents and to distinguish to sounds of different alarms.
