Skilled Nursing Home - Outside Visitation Guidelines

Luther Memorial Home is now able to offer outside visitation to skilled nursing home residents.  The following guidelines must be met for you to be allowed to visit:

  • Visits must be scheduled
    • LMH's activity Department will oversee the scheduling of visitations
      • These visitations will occur in the LMH Courtyard
        • As the visits are occurring outside, each visit will be weather permitting
        • If inclement weather does occur, the visitation will need to be reschedules.
    • These visits can occur Monday - Friday
    • 10am - 11am and 1pm - 3pm
    • You must contact the activity department Mon - Fri, 8am - 4pm to schedule your intended visit.  This contact must be made the business day prior to the visit.  The activity department will then inform you if a visit at that point in time is possible.
    • Face to face outdoor visits will be scheduled for 20 minutes
      • For those of you who want to visit for longer than the assigned time frame, you will then need to resort to a window visit
  • Each visit will have a designated area.
    • LMH will have pre-assigned locations created to allow outside visits.
    • Minimum of 6ft distance between resident and guest at all time.
    • At no time is physical contact permitted.
    • A limit of 2 guests per resident at a time.
    • If these guidelines are not followed, LMH will then revoke outside visitation privileges for that guest - not the resident
  • LMH will have staff supervising the general area to ensure each resident and guest are abiding by LHM's visitation rules
  • Masks must be worn at all times during the visitation by both resident and quests.

 

Check-in procedure

  • Guests must check in at the front lobby of LMH.
  • Guests must check-in 15 minutes prior to their visitation slot.
  • Hand hygiene must be conducted upon arrival in to the building.
  • Guests are required to wear a mask at all times on LMH premises.
    • Guests are responsible to bring their own masks.
  • Guests will them be required to complete a screening  process.
    • If an individual is exhibiting any signs or symptoms of COVID-19, have been recently exposed, or tested positive, you will not be able to visit at this time.
  • If the guest(s) follow all check-in procedures, the guest(s) will then be re-directed to the outside visitation area.
