Admission Procedure Luther Memorial Home

  1. Application: Needs to be turned in to Social Services PRIOR to admission. LMH needs this information in order to prepare for the new resident. The application can be dropped at LMH, mailed, sent via email to strosen@lmhnd.org or faxed to us at 701-786-9022.
  2. Nursing Visit: A Nurse will come out to assess a potential resident for admission, in their own home or at the hospital, prior to admission to ensure we are able to meet this person’s needs.
  3. History & Physical: A History and Physical (H&P) must be done within five days prior to admission. We can assist you in making this appointment with the physician of your choice and provide the paperwork to the Dr. that needs to be completed.
  4. Admission Times: LMH admits new residents at 10AM, Monday through Thursday. Admissions are not done on weekends.

There are exceptions to the admission process so please don’t hesitate to call with questions or if you need clarification on the above information.

Social Services is available to give tours of LMH and answer questions. The Social Service office is open Monday through Friday from 8AM to 4PM.
